The dermis consists of protein fibres and the extracellular matrix. Hyaluronic acid (HA) is an abundant glycosaminoglycans a natural compound of the dermal extracellular matrix that has a high affinity to water. This characteristic is determinant of the skin’s viscoelasticity, hydration, structure, and firmness.
During the ageing process, collagenesis and elastogenesis are diminished and there is an increase in the disorganisation and degradation of dermic fibres and a reduction in glycosaminoglycans. This results in a reduction of elastic, density, and resistance of the skin.
Skin boosting treatments are a new concept that promotes a global improvement of the skin through a series of hyaluronic acid injections into the dermis. The objective of this approach is to increase skin hydration, delaying and reversing the ageing process and reducing ageing indicators. It can be used by clients of any age to help hydrate the skin and reduce the onset of ageing.
After hyaluronic acid is injected into the dermis an improvement in hydration, and the subsequent stimulation of collagen production.
Types I and III collagen are found in the dermis with the major structure protein being type I. Photodamaged skin exhibits degradation of these structures and their precursors, by metalloproteinases produced by fibroblasts, keratinocytes, and inflammatory cells.
The extracellular matrix and the connective tissue is constituted by endogenous polysaccharides and glycosaminoglycans. Among them is hyaluronic acid which is found in the highest concentrations in the dermis and not linked to proteins. Besides having many functions, hyaluronic acid can bind water up to 1000 times its own volume, and this characteristic maintains the skins hydration and turgor and is detrimental for the skin’s viscoelasticity and firmness.
The global ageing process, both photoinduced (extrinsic) and genetic (intrinsic), involves diminished collagenesis and elastogenesis, dermal fibre disorganisation and degradation and a decrease in glycosaminoglycans. Due to this, aged skin tends to be thinner, less dense, less distensible, and less resistant. Once the mechanical tension is diminished it leads to collagen degradation which contributes to the ageing process.
Once hyaluronic acid is injected into the dermis, with its consequent water binding, fibroblasts are stimulated which results in an increase in collagen synthesis, especially type I. It is believed that a mechanical stretching of fibres in the extracellular matrix enhances the interaction with fibroblasts, raising their collagen production. Cutaneous density increases leading to more firm, smoother skin.
Hydration can help to keep the volume and elasticity of the skin, but the challenge is how to maintain this hydration with hyaluronic acid. Topical application of hyaluronic acid is not as effective as proper penetration is not always possible due to its large molecule size. The alternative way to effectively achieve a significant increase is hyaluronic acid in the dermis is to inject it into the skin delivering the polysaccharide directly into the dermis.
